This addendum supplements the terms of our general Privacy Notice and does not apply to any information subject to the Health Insurance Portability and Accountability Act (HIPAA), the California Confidentiality of Medical Information Act (CMIA), or any clinical trial data subject to other federal laws.
PROCEPT BioRobotics, for the purpose of the business online or offline, collects and uses the following categories of personal data*:

PROCEPT BioRobotics collects these categories of personal data from the following sources:
- Directly from the individual the information is about
- Indirectly from the physician or health institution that the individual interacted with
- Indirectly from an individualās interaction with our website or other online tools
We use and disclose personal data in accordance with the purposes established in the sections
Sharing or Selling of Personal Data
Under the applicable U.S. laws, PROCEPT BioRobotics engages in selling or sharing of personal information to third-parties for cross-context behavioral or targeted advertising. - Right to Opt-Out of Sale, Sharing, or Targeted Advertising ā You may have the right to opt-out of selling, sharing, or processing of your personal information for cross-context behavioral advertising or targeted advertising, as those terms are defined under applicable law.
- Right to Limit Use and Disclosure of Sensitive Personal Information ā If we use or disclose sensitive personal information for purposes beyond those permitted by applicable law, you may have the right to direct us to limit such use and disclosure.
- Right to Withdraw Consent ā Where we rely on your consent to process certain personal information, including sensitive personal information where required and as defined by applicable law, you may have the right to withdraw that consent.
- Right to Obtain Information About Third Parties ā In some states, you may have the right to obtain a list of specific third parties, or categories of third parties, to whom we have disclosed your personal information.
- Right to Appeal ā If we decline to take action on your request, you may have the right to appeal our decision in accordance with applicable law. We will provide instructions for submitting an appeal in our response.
- Right to Non-Discrimination / Non-Retaliation ā You may have the right not to receive discriminatory or retaliatory treatment for exercising your privacy rights.

We will confirm receipt of your request in accordance with applicable law. We endeavor to respond to verifiable consumer requests within the timeframes required by applicable law.
Our ability to respond to your request may depend on our ability to verify your identity. We may request additional information from you to verify your identity and process your request. If we are unable to verify your identity, we may deny your request in accordance with applicable law.
You may designate an authorized agent to submit a request on your behalf where permitted by law. To do so, the authorized agent must provide proof of authorization, such as written permission signed by you or a valid power of attorney. We may also require you to verify your identity directly with us and confirm that you provided the authorized agent permission to act on your behalf. We may deny a request from an authorized agent if we are unable to verify your identity or the agentās authorization, as permitted by applicable law.
