Consent to Use of Voice and Likeness

I understand and agree that the Webinar may be recorded by PROCEPT, including any questions and discussion from participants. I hereby give PROCEPT my consent to use, free of charge, my name, audio, text, content, likeness and other personal information on its website, online services, emails, social media, and third-party media (publicly available worldwide) for any legal purposes including product marketing and educational recordings. I expressly release PROCEPT and its representatives from and against any and all liability or any cause of action arising out of the use of my likeness, name, or voice including any claim under California Civil Code Section 3344, or any equivalent law or regulation in the jurisdiction in which I reside. I understand that if I do not wish to have my name, likeness and personal information used, I can simply not enter my personal information for display into the webinar, not activate my camera, and not share my voice; I can ask questions via chat feature without giving my name.
